Male Rhinoplasty

defined profile with strong and bold lines

Rhinoplasty is one of the most common procedures requested by males

Candidates for male rhinoplasty are patients:
  • Who have a thick skin and fat around the alar
  • With a widened nose
  • With a bulbous nose
  • With large nostrils
  • Who desire an increase in tip projection

Male rhinoplasty is different from female rhinoplasty

The goals of a rhinoplasty are different for men and women. Women tend to want to have smaller noses, with delicate and well defined nose tips, whereas for men, they want the nose to be straight and to have sufficient structure and volume.
  1. The angle between the forehead and nose bridge should be around 130-140 degrees
  2. The width of the nose tends to be wider to prevent it form looking too feminine (10mm)
  3. The dorsal aesthetic lines should be straight
  4. The angle between the nose and the upper lip should be 90 degrees
  5. The nose tip should be well-defined but also sufficient volume and wider than that of a female

Common issues encountered in Male Rhinoplasty

Some of the common issues discussed by males include low nose, hump nose, crooked nose and droopy tip

Dorsal Augmentation

Having a low nose bridge can create a dull appearance. The flat nose is one of the first priority to correct to create a strong, masculine look. This is in contrast to the curved nose line that we design for females. In male patients, it is important to ensure that the tip of the nose matches the elevated nose bridge to prevent a drooped tip.

Augmentation of the nasal dorsum can be done with silicone implants or autologous cartilage grafts. Using spreader grafts, we are able to widen the width of the nasal dorsum to the ideal width of 10mm and also to create perfect straight lines. The nasal tip has to be well projected and also of sufficient volume in contrast to a petite defined nose of a female. This is achieved with the use of onlay tip grafts taken from the ear and meticulous suture techniques.

Osteotomy to correct hump nose

The nasal hump can be due to congenital reasons or as a result from traumatic sports injury or altercations. It can also be associated with a crooked nose and breathing difficulties.

Correction of the hump nose helps to create a refined, gentlemanly appearance. This irregularity could be at the level of the bone or cartilage and it can either be camouflaged or removed carefully. Upon removal of the hump deformity, the dorsum of the nose can be smoothened with autologous tissue such as fascia or diced cartilage and fascia to give a smooth appearance.

Correction of Deviated Nose and Improved Breathing

A deviated or crooked nose can be congenital or as a result of trauma. This can create an unrefined appearance and could also be associated with breathing difficulties. It is absolutely essential that the deviation of the nose is diagnosed before surgery to prevent complications such as recurrence or possible deviation in the future.

The curved septum is removed and the remaining septum is reinforced with spreader grafts which help to create a wider and straight dorsal profile. The septum extends to the lower cartilages of the nose and alignment with the anterior nasal spine helps to ensure that the tip is also straight. Setting the septum straight creates the foundation necessary for any tip work and also creates the support structure to project the nasal tip.

Tip-plasty

Reshaping of the tip is done at the same time as dorsal augmentation. This helps to ensure that the elevated nasal dorsum fits the rest of the nose and is harmonious with the rest of the appearance. The dorsal aesthetic lines run from the forehead along the brow down the side walls of the nose to the tips of the nose.

A masculine nose tip has to have sufficient volume, but not bulbous and be well defined. The tripod structure of the tip has to be strong and robust to support the volume of the tip. Natural tip-plasty is performed with the use of autologous grafts, usually taken from the ear cartilage and stitched meticulously with the use of well executed suture techniques to create a nasal tip that is secure and rigid so as to prevent recurrence of the nasal droop.

Understanding what happens during Male Rhinoplasty

Correction of deviated septum + hump deformity + Dorsal Augmentation + Tip Plasty

01  Firm foundation

Correction of nasal deviation or hump deformity

It is important to ensure that the foundation for rhinoplasty is good and symmetrical. Any deviated septum is removed and spreader grafts are used to reinforce the septum, creating a wider dorsum and also clean, straight dorsal aesthetic lines.

Hump deformity is corrected to prevent any deviation or recurrence in the future. The cartilage or bone is removed in gradual increments. This smooth profile forms the base for dorsal augmentation.

02 Dorsal Augmentation

Dorsal Augmentation to create the ideal angle

The ideal angle between the forehead and nose is 130-140 degrees and from the nose to the lips is 90-100 degrees for males.

The angle between the forehead and the nose can be created using fascial grafts or silicone implant and the nasal septal angle can be defined with the septal extension grafts or cartilage grafts to create a suitable nasolabial angle.

03 Reshape the tip

Definite tip features with adequate volume

In male rhinoplasty, unlike female rhinoplasty, the nasal tip has to maintain sufficient volume and structure to exert its masculinity.

Using cartilage grafts and suture techniques we are able to create a tip that suits the augmented nasal dorsum.

Getting Male Rhinoplasty Right

01

Ideal Nose Height

The ideal height of the nose is important to create the angle between the forehead and the nose. This creates a masculine and sophisticated look. At the same time, it is important to correction the nasal deviation or hump nose irregularity.

02

Ideal Tip Projection

To create the masculine look, the tip of the nose has to project sufficiently to as to prevent having a droopy tip when the dorsum of the nose is elevated. This has also got to be balanced with the overall harmony of the face.
