During open incision blepharoplasty, the double eyelid crease is designed, the incision is made on the eyelids, the muscle and fat is removed and the skin and muscle is then tied down to fixate the new crease.

  • Incision blepharoplasty sounds scary and painful and can leave you with "sausage eyes:

    The open incision and non-incision methods should have similar results. The incision method is chosen for selected patients and has more pros than cons, it can create a wide variety of lines and produce a distinct eye shape.

  • Incision blepharoplasty always leaves a mark and there are tell-tale signs of surgery

    The incision technique if well planned and well executed will land the new scar in the eyelid crease giving natural-looking lines that feel like you've always had them.

Incision is done only when necessary

Most patients do not prefer the incision method and neither do we. However, some reasons where Incision Blepharoplasty is preferred include: 

  1. Excess skin (droopy skin)
  2. Excess and bulky fat
  3. Previously failed suture blepharoplasty
  4. Ptosis correction (click here for more information)
  5. Distinct eye shape and wide variety of lines

Open incision allows for complete control over eyelid crease and shape of the fold.

In-side crease or Tapered crease 

The crease begins from the inner corner of the eye and fans outwards giving a more subtle or natural appearance.

Out-side crease or Parallel crease 

The crease does not intersect with the upper eyelid margin and it fan-shaped or parallel (as shown) to give a more more extroverted, elegant and expressive appearance. This may not be suitable for all Asian eyes.

Fan-shaped crease or Hybrid crease 

The crease begins low from the inner corner of the eye without intersecting the eyelid margin and and fans outwards giving a more subtle or natural appearance.

TAO Suture technique 
creates permanent and reliable creases 

TAO Suture

Finding the balance between scar permanence and a scar that is not noticeable is the key. Scar fixation is important which is why we use the Tarsus-Aponeurosis-Orbicularis (TAO) fixation technique. This gives the surgeon absolute control over the scar. 

Patients are often concerned regarding scarring, swelling, and sausage eyes. The scarring is usually not an issue as the incision is hidden within the natural eyelid crease or the new crease. The swelling also reduces with time. Most patients are back to work by Day 5 and most of the swelling disappears by Day 14, with all of it gone after 6 months.

"Open incision blepharoplasty using the TAO suture is a powerful technique that can create a wide variety of eyelid lines. This is reserved for the selected patient that would not be a good candidate for non-incision blepharoplasty or had previous failed surgeries. Open incision technique can create more permanent creases altering the existing anatomy and fold."
