Understanding Different Eyelid Creases

HIGH CREASE

Large eyelid folds (distance between eyelid crease and eyelid margin) are compatible with thin skin and large palpebral fissures, whilst small folds are more natural for thicker skin and smaller apertures.

HYBRID CREASE

Fan-shaped folds with a combination of fan-shape and parallel are narrow at the medial canthal area with creases that run parallel in the lateral portion of the eyelid. My personal preference is the elongated inside fold with a narrow parallel fold.

INSIDE CREASE

Inside folds are fan-shaped, whereas outside folds can be fan-shaped or parallel. I prefer the inner fold to start as medial as possible to open up the palpebral fissure to give a more sophisticated look. The inside or tapered crease is more natural and subtle, more suitable for an introverted individual.

OUTSIDE CREASE

Outside folds or parallel crease refers to upper eyelid creases which do not intersect the upper eyelid margin. Outside folds can be fan-shaped, parallel, or a combination of the two. Parallel creases maintain an even, fixed distance to the upper eyelid margin from medial to lateral canthus. Outside folds are often interpreted as extroverted, elegant, and more expressive. This may not be natural for all Asian patients.

Suture blepharoplasty is a minimally invasive procedure that achieves a similar attractive double eyelid look as incision upper blepharoplasty but without requiring a long incision. Fine permanent sutures are used to create the crease, making it particularly suitable for individuals with thinner eyelids and those desiring a shorter recovery period.

Incisional blepharoplasty creates a permanent double eyelid crease by making a direct incision on the upper eyelid. Pre-Surgical Guidelines

You will be advised to discontinue the use of supplements that may thin the blood and alcohol one week before surgery, and stop wearing contact lenses 3 days before. Smoking cessation should begin 2 weeks before the procedure to enhance recovery. On the day of surgery, ensure all eye makeup, false lashes, and eyelid tape are removed.

Step-by-Step Process

1. Anaesthesia Administration

Local anaesthesia or light sedation is typically employed to ensure your comfort during the procedure. While the initial injection may cause slight discomfort, the area will quickly become numb. Any mild discomfort at the injection site is brief and subsides quickly, allowing you to remain relaxed throughout the procedure.

2. Marking the Eyelids

Key reference points are marked to guide incision placement. These markings take into account individual facial features and the desired crease position, ensuring symmetry and natural-looking results.

3. Surgical Technique

Surgical techniques include suture blepharoplasty, where sutures are placed to create a fold using small incisions that typically heal without visible scars. In crease creation surgery, incisions are made to remove excess skin, fat, or muscle, followed by sutures to form a permanent crease.

Additional procedures such as medial epicanthoplasty, lateral canthoplasty, or sub-brow lifts may be performed as needed. The surgery is performed as a day procedure using loupe magnification to enhance accuracy, with Ethilon 7/0 fine sutures utilised to achieve optimal results.

4. Closure and Dressing

After the procedure, any incisions made are meticulously closed, and the area is protected with a light dressing or ointment. Employing meticulous suture techniques promotes better healing and minimises scarring.

Post-Surgical Care and Recovery

Initial Recovery

Patients may experience soreness, tightness, and swelling, with maximum swelling typically occurring 48 to 72 hours post-surgery. Cold compresses should be applied during the first 48 hours to minimise swelling. Clean the eyelids gently with gauze and sterile saline three times daily and apply antibiotic ointment to the incision line to prevent infection. Pain relievers should be taken as prescribed to manage discomfort.

Activity Restrictions

To ensure proper recovery, refrain from smoking, alcohol consumption, strenuous exercise, and upper eyelid makeup for at least 2 weeks. Avoid eye or facial massages and any activity that could put pressure on the eyes during this period.

Recovery Timeline

Swelling and bruising typically resolve within 1 to 2 weeks. Sutures are usually removed 5 to 7 days after surgery during a follow-up appointment, depending on the patient’s healing progress. Most patients can resume light activities after 2 weeks, with the final results, including the eyelid’s appearance and function, becoming fully apparent within 2 to 3 months.

Potential Risks and Considerations

While complications are rare, potential side effects include bleeding, infection, and scarring. Excessive bleeding may require medical intervention, and infections might need antibiotics or additional surgery. Scarring varies in visibility, with treatments like scar cream and sunblock offering improvement. Mild asymmetry or aesthetic concerns may arise, sometimes necessitating revision surgery after swelling subsides. Temporary functional issues, such as dry eyes, eye injuries, allergic reactions, or rare anaesthesia-related complications, are also possible.

What is the difference between incisional and non-incisional methods?

The incisional method involves a small incision along the eyelid crease to remove excess skin, fat, or muscle, making it suitable for thicker or droopy eyelids. Non-incisional results can be adjusted by removing or adjusting the permanent sutures if done early. Incisional surgery is more challenging to reverse since tissue is permanently altered. However, revisions can be performed by an experienced surgeon to modify or correct the results.

Can this surgery help with functional issues like eyelid heaviness or ptosis?

Yes, eyelid surgery can address functional issues such as eyelid heaviness and ptosis. By removing excess skin or fat, it can reduce the feeling of heaviness and improve vision obstructed by sagging skin. How is the crease height and shape determined?
