Types of Incisions

Incisions, as small as 2.5 cm depending on implant size, are chosen based on anatomy, implant type, and desired results. Each option minimises scarring while ensuring precise placement.

Transaxillary Incision: Located in the underarm, this incision avoids scars on the breast. It is a good option for those preferring a scar-free chest, though it is often used for smaller implants.

Periareolar Incision: Placed along the lower edge of the areola, this incision blends naturally with the skin’s colour transition. It offers good access for implant placement and is ideal for those concerned about visible scarring.

Inframammary Incision: Made in the fold beneath the breast, this incision provides direct access for precise implant placement. The scar is well hidden in the natural crease, making it suitable for most implant types and sizes.

Preparing for the Procedure

Medical Assessment

A physical examination evaluates overall health status and breast tissue characteristics. Blood tests check for any underlying conditions that may affect surgery or recovery. A pre-operative breast ultrasound or mammogram is suggested before breast surgery.

Medication Adjustments

Supplements and medications that can affect clotting, including aspirin and certain anti-inflammatory drugs, need to be stopped 2 weeks before surgery.

Pre-operative Guidelines

Smoking cessation is required at least 2 weeks before surgery to promote proper healing. Alcohol consumption should be avoided the day before surgery. No food or drink is allowed for 6 hours before surgery.

Use of the Keller Funnel
Breast Augmentation

The keller funnel is a simple yet ingenious contraception that allows deployment of the breast implant with a no-touch technique to further enhance sterility whilst enabling the surgeon to squeeze the implant through a smaller incision.

Depending on the size of the implant, the incision can be as small as 2.5 cm to 3cm.

Use of specialised instruments

The use of a lighted endoscope and specialised instruments allows the operation to be performed safely through a small incision while ensuring that the breast implant pocket is accurately dissected and that any bleeding is stopped to reduce bruising.

Surgical Process

Incisions are made in the pre-determined areas, such as under the breast, around the areola, or in the armpit. A pocket is then created either beneath the pectoral muscle (submuscular) or directly behind the breast tissue (subglandular) to accommodate the implant.

Closure and Dressing

Once the implants are placed, the incisions are closed with sutures. We use a triple suture technique to anchor the incision so that it does not migrate. Adhesive strips or surgical tape are applied to support healing, and the area is covered with a silicone dressing, which can be left covered for a week.

Post-Surgical Care and Recovery

Immediate Post-Surgery

After the procedure, you will be monitored in a recovery area until the effects of anaesthesia wear off. Your breasts will be supported by an elastic breast band and surgical bra to minimise swelling and aid healing. The dressing will be kept until the next review so you will not need to expose the wound.

Swelling Management

Swelling and discomfort are normal in the initial stages and will gradually subside. Medications may be prescribed to help manage pain and reduce the risk of infection.

Recovery Process

Light activities can usually be resumed within a few days, with most patients returning to work within 1–2 weeks, depending on the nature of their job. Strenuous activities, including heavy lifting, should be avoided for at least 4 weeks.

Aftercare Recommendations

Wearing a support bra as directed can help maintain proper breast positioning during healing. It is also necessary to keep the incision sites clean and dry and follow all wound care instructions. Avoid smoking for at least two weeks after surgery, and attend all follow-up appointments to monitor your progress.

Potential Risks and Complications

Breast augmentation carries some risks, which may include capsular contracture, where scar tissue forms around the implant; implant leakage or rupture; and changes in nipple or breast sensation, which may be temporary or permanent. Other potential complications include infection and bleeding as well as aesthetic concerns such as skin wrinkling over the implant. In some cases, revision surgery may be required to address these issues. These risks will be discussed during the consultation, along with strategies for effective management.

Frequently Asked Questions (FAQ)

What is the lifespan of breast implants?

Breast implants are not lifetime devices. While most last between 15 and 20 years, this varies based on individual circumstances, and monitoring through regular check-ups is recommended.

Will I lose nipple sensation after breast augmentation?

Sensation changes are possible after surgery. These changes are usually temporary but may be permanent in some cases, especially if the incision is around the areolar.

What happens if I want to remove my implants later?

Implant removal is possible, and the breasts can either be left as is or reshaped with additional procedures such as fat grafting, depending on your preference.

Do breast implants increase the risk of breast cancer?

There is no evidence that breast implants cause breast cancer. However, implants can affect mammogram imaging, so it is necessary to inform your radiologist about them.

Can breast augmentation fix sagging breasts?

While breast augmentation enhances size and volume, it does not address sagging. A breast lift may be recommended in combination with augmentation for a more comprehensive improvement.
