Types of Fillers

Hyaluronic Acid (HA) Fillers

HA is a naturally occurring substance in the skin that provides hydration and volume. Brands like Restylane, Juvederm, and Belotero are commonly used to treat lips, cheeks, fine lines, and under-eye hollows. Results typically last 6–18 months.

Calcium Hydroxylapatite (CaHA) Fillers

Known for its thicker consistency, this filler stimulates collagen production for long-lasting effects. Radiesse is a known brand, ideal for deep wrinkles, cheek augmentation, and hand rejuvenation, with results lasting up to 18 months.

Poly-L-Lactic Acid Fillers

These fillers, such as Sculptra, work by gradually stimulating collagen production. They are best suited for areas with significant volume loss, such as the temples and cheeks. Improvements occur over several months and last for over two years.

Polymethylmethacrylate (PMMA) Fillers

Containing tiny biocompatible microspheres, PMMA fillers like Bellafill offer semi-permanent to permanent correction. These are used for treating deep wrinkles and acne scars, providing long-term results.

Preparation for Filler Injections

Consultation and Assessment

A pre-treatment consultation involves evaluating facial anatomy, medical history, and aesthetic goals to develop a treatment plan. Skin quality is assessed, and appropriate injection sites are determined.

Pre-Treatment Guidelines

It is recommended to avoid blood-thinning medications, supplements (such as fish oil, vitamin E, garlic), and alcohol for 7-14 days before treatment. Specific instructions may be provided based on individual conditions.

Allergy and Sensitivity Checks

For non-hyaluronic acid fillers like Radiesse or Sculptra, allergy testing may be necessary. A review of any history of allergic reactions is also recommended.

Skin Health

Treatment areas must be free from active infections or inflammatory conditions. Any skin conditions should be resolved before filler injections.

The Filler Injection Process

1. Preparation

The treatment area is cleansed with an antiseptic solution, and a topical anesthetic is applied for 15-30 minutes. Injection points are then marked.

2. Injection

The filler is injected into the desired areas with a fine needle or cannula. Multiple entry points ensure even distribution.

3. Shaping

The treated area is gently massaged to distribute the product evenly, ensuring smooth contours and preventing lump formation for natural-looking results.

4. Completion

The results are evaluated, and aftercare instructions are provided to support proper healing and optimal outcomes.

Aftercare and Recovery

Immediate Aftercare

Mild redness, swelling, and bruising are common after treatment. Applying cold compresses can help reduce swelling. Avoid touching, massaging, or applying pressure to the treated areas as improper handling may affect filler placement.

Activity Restrictions

Refrain from strenuous exercise, excessive sun exposure, and heat sources like saunas for at least 48 hours. Postpone facial treatments and dental procedures for two weeks. Avoid alcohol during this period to minimise swelling.

Recovery Timeline

Swelling and bruising typically subside within 3-5 days. Final results are visible as filler settles, usually within 7-14 days. Schedule follow-up visits as recommended to evaluate the results and address any concerns.

Potential Risks and Complications

Filler injections are generally safe but can have side effects like swelling, redness, bruising, or tenderness, which typically resolve within a few days. Rare risks include infection, lumps, asymmetry, filler migration, nerve damage, or vascular issues such as tissue damage. Allergic reactions, though uncommon, may range from mild irritation to severe.

Frequently Asked Questions (FAQ)

Are filler injections painful?

Most patients experience minimal discomfort during treatment. Topical numbing cream, local anaesthesia, and fillers containing lidocaine help manage any pain.

Can filler injections help with acne scars?

Certain fillers can improve acne scars, particularly deep, rolling, or boxcar scars. PMMA fillers like Bellafill are FDA-approved specifically for acne scar treatment, while hyaluronic acid fillers may also help depending on scar type.

Are filler injections safe during pregnancy or breastfeeding?

Dermal fillers are not recommended during pregnancy or breastfeeding. No clinical studies have established their safety during these periods, and practitioners typically advise waiting until after breastfeeding is complete.

What happens if I don’t like the results?

Hyaluronic acid fillers can be dissolved using an enzyme called hyaluronidase. This provides a reversal option if the results are unsatisfactory. Non-HA fillers cannot be dissolved and must naturally metabolise over time.

Can fillers migrate to other areas after injection?

While rare, filler migration can occur. Risk factors include incorrect injection technique, excessive movement of treated areas, or improper filler placement. Choosing a qualified practitioner can help to reduce these risks.
