A wide alar base and bulbous tip can give the nose a flat, undefined appearance, contributing to a less refined visage.

The alars are the fleshy sides of the nostrils, and when widened, they can make the nose appear disproportionately large and out of harmony with the facial fifths.

Alar-plasty involves narrowing the alar base to reduce the width of the nose, restoring balance and proportion. Nostril-plasty can further refine the nostrils by reducing their size and reshaping them.

Considering the Face from Every Angle

Rhinoplasty requires precision and careful consideration of facial harmony.

The nose, as the central feature of the face, must be in proportion to other facial features for optimal aesthetic results.

Using 3D imaging, we perform detailed planning and take precise measurements to ensure a balanced and natural outcome. Ideally, the width of the nose should match the width of the eye. Excessive nostril flare can disrupt this balance, giving the nose a flat appearance, which can be addressed during surgery.

This approach ensures that the nose complements the face from all angles.

Open Incision Technique

The open incision technique involves a wedge incision to narrow the alar base by 2–4 mm, with the incision carefully hidden along the natural alar groove to ensure scars are not visible.

This approach narrows the alar base, reduces nostril size, and elevates and projects the nasal tip for a more balanced appearance. However, it is not suitable for patients with naturally narrow nostrils, as it may cause breathing difficulties. It is particularly effective for individuals with larger nostrils, as it allows precise control over nostril size and nasal proportions.

Non-incision Technique

The non-incision technique uses alar sutures to draw the edges of the nose together and narrow the columella without removing any skin.

This approach is ideal for patients with a wide columella and normal-sized nostrils. However, it is not suitable when nostril size reduction is required, as there is a higher risk of recurrence.

Key benefits include the ability to adjust the width of the alar and columella, a fast recovery, no visible scars, and no risk of nasal congestion.
